Prolactin on TRT and PED Cycles – When Is It a Problem?

Prolactin is a pituitary hormone that receives far less attention than testosterone, estradiol, and DHT, but elevated prolactin can significantly affect libido, erection quality, orgasm, fertility, and overall sexual function in men. For men using TRT or performance-enhancing drugs, an elevated prolactin result should be investigated rather than automatically blamed on testosterone.

Does TRT Increase Prolactin?

Testosterone replacement therapy itself is not generally recognized as a cause of hyperprolactinemia. One of the strongest long-term datasets comes from Yassin et al., who followed 321 hypogonadal men receiving testosterone undecanoate for up to 12 years. Testosterone, free testosterone, estradiol, SHBG, LH, FSH, progesterone, and prolactin were followed throughout treatment. Despite substantial changes in several hormones, prolactin was essentially unchanged—approximately 11.0 µg/L at baseline versus 10.8 µg/L after 12 years. This provides strong evidence that long-term testosterone therapy does not inherently increase prolactin.

Prolactin and 19-Nor Compounds

Prolactin becomes a greater consideration when PED protocols include 19-nor anabolic steroids such as nandrolone (Deca) or trenbolone. These compounds have progestogenic activity and can alter the hormonal environment in ways that may contribute to sexual dysfunction and, in some users, elevated prolactin. That relationship is often oversimplified, however. Human evidence directly showing that nandrolone or trenbolone consistently causes hyperprolactinemia is limited—particularly for trenbolone, where controlled human data are scarce. Elevated prolactin during a 19-nor cycle should therefore be confirmed with laboratory testing rather than assumed from symptoms alone. Symptoms such as reduced libido or erectile dysfunction during nandrolone or trenbolone use can also result from changes in estradiol, androgen balance, neurosteroid signaling, or other factors.

What Commonly Causes High Prolactin in Men?

When prolactin is genuinely elevated, three major categories should be considered:

  1. Medications and drugs. Dopamine normally suppresses prolactin secretion. Drugs that interfere with dopamine signaling—including certain antipsychotics, anti-nausea medications, opioids, and some other medications—are among the most common causes of non-tumor-related hyperprolactinemia.

  2. Pituitary or hypothalamic disorders. A prolactinoma can directly produce prolactin, while other pituitary masses can elevate prolactin by interfering with dopamine delivery to the pituitary.

  3. Endocrine or systemic disease. Primary hypothyroidism, kidney dysfunction, and certain other systemic illnesses can increase prolactin. These should be excluded before assuming that TRT or a PED protocol is responsible.

Stress, strenuous exercise, sexual activity, sleep, and even a difficult blood draw can also temporarily increase prolactin. A mildly abnormal result often warrants a properly controlled repeat measurement before treatment is considered.

Treating Elevated Prolactin

Treatment should address why prolactin is elevated, not simply reduce the laboratory number. When treatment is appropriate, dopamine agonists can markedly suppress prolactin. Cabergoline is the preferred dopamine agonist for prolactinomas in current endocrine guidelines because of its effectiveness and relatively long duration of action. Other dopaminergic drugs, including pramipexole, can also suppress prolactin, although pramipexole is not a standard first-line treatment for hyperprolactinemia. These medications should not be used casually simply because someone is taking a 19-nor compound. Dopamine agonist use should be kept to the minimum necessary and closely monitored. Excessive suppression of prolactin can create its own problems, while dopamine agonists can produce adverse effects including nausea, dizziness, orthostatic hypotension, sleep disturbances, and behavioral or impulse-control changes.

The Bigger Picture

On conventional TRT, an unexplained elevation in prolactin should prompt investigation for medications, pituitary disease, thyroid dysfunction, kidney dysfunction, and other established causes rather than automatically attributing it to testosterone. During PED cycles, particularly those containing nandrolone or trenbolone, prolactin deserves closer monitoring, but laboratory data should guide decisions rather than assumptions based solely on the compound being used.
